New Patient Information

At Campus Walk Dental Care, our goal will always be to treat our patients with the respect they deserve and provide them with a warm and welcoming environment where they will receive the highest quality of dental care available. We are proud to work with patients of all ages in Durham, NC and the surrounding areas.

By filling out these forms ahead of time you will save significant time on your visit. If you have any questions when filling out the form, please do the best you can, and our staff will assist you with your questions on the day of your appointment.

If you have a printer at home and rather than filling out all the forms digitally, you can click on following tabs, print the forms, fill them out at home and bring them to our office.

Please arrive 15 minutes before your scheduled appointment time to ensure all the required forms are completed before you step into our operatory. This way, we can fully utilize the allotted time for your dental care.

Your First Appointment

To provide you with prompt dental treatment, please be sure to bring the following items when you visit our practice for your initial visit:

  1. Insurance Information/Card
  2. Photo Identification
  3. Any medical/dental records pertaining to your visit including any x-rays, other imaging, labs and studies if applicable
  4. A list of medications you are presently taking

Insurance Information

We accept most dental insurance plans. Please familiarize yourself with your insurance plan; paying close attention to your benefit maximums, deductibles, and covered percentages.

As a service to our patients our staff will:

  • Prepare all the necessary forms for your dental benefits
  • Bill your insurance company for you and help you with any claim disputes.

However, we remind you that your specific policy is an agreement between you and your insurance company and it is your responsibility to review and to know your insurance plan.

Payment Information

Payment is expected at the time services are rendered, unless other arrangements have been made prior to the service. If you have dental insurance coverage, this includes any co-payments or deductibles and fees over your yearly maximum–as well as non-covered services.

We accept the following forms of payment: cash, check, Visa, MasterCard & Discover, as well as financing through CareCredit.
